Open Out believes that a solid aftercare plan is a quintessential part of maintaining long-term recovery.
What is the Aftercare Program?

A client’s treatment program at Open Out provides a great starting point and a solid foundation to start the journey of long-term recovery. However, it takes time and a lot of hard work to develop a strong program of recovery. This is where a customized aftercare plan plays a very important role and our therapists are dedicated to helping our clients create this plan as they approach their discharge date. Local peer-support groups, 12-step meetings, local therapists, and other local recovery resources are located and shared with our clients and they are strongly encouraged to get involved in their local recovery communities. Clients will also be contacted regularly by one of our alumni coordinators to see how they are progressing in their recovery. Follow-up phone or video conferences with the client’s primary therapist can also be arranged. At Open Out, we want the best for our clients and we are committed to helping them achieve long-term recovery in any way we can.

How is the aftercare plan customized to each client?
Our client’s customized aftercare plan is based on their individual needs and home location and includes recommendations for clinicians and case management referrals if they have needs that require it. After we have assessed the client’s needs, we have virtually limitless access to resources that allow us to customize each client’s aftercare plan to meet their needs and set them up for success.
List of items for an aftercare / discharge plan
Anything that can help a client maintain a solid recovery plan can be included in this list including, meeting lists for 12-step programs, halfway house referrals, and outpatient programs. All of these have been regarded as appropriate for anyone in early recovery.
What are the goals of the aftercare plan?
A client’s continued recovery is the ultimate goal of any aftercare plan. The plan gives the client a detailed hard-copy of resources that will help them to achieve this goal.
